1. Need to fill Online Application form [ DOWNLOAD ] and take an appointment for submission. 2. Valid passport for 9 Months (Copy of the Passport - 2 copies of first page on a separate sheet, one copy of last page). 3. Two Recent Photographs, 35mm x 45mm with white background, 80% face (Photographs should not be older than 6 Months). 4. Covering letter on company letter head addressing to the Poland consulate Mumbai with Company Profile. 5. Invitation letter with the passport details of the Applicant addressing to the Poland Consulate, Mumbai fax directly to the consulate. 6. Overseas Medical Policy. 7. For Proprietor - Required Company and personal Bank Statement. 8. For Proprietor - Required Company's ITR-5. 9. For Proprietor - Required Company's Registration Copy. 10. Air ticket. 11. Salary slips for the last 3 months. 12. IT papers of last 3 years . 13. Original Latest Bank statement for last 6 Months. 14. Appointment letter copy. 15. International credit card. 16. Hotel Confirmation from Poland.
